Applications for membership must be made to the Executive Director in writing on an application form provided and accompanied by the following:
- Remittance by credit card, check, money order or bank draft in United States currency, drawn on a U.S. bank, covering one year's annual dues in advance plus the application fee applicable at the time of the application. Credit card details may be listed on the bottom of the application or submitted separately. Funds may also be wired through Western Union. Please contact us for instructions.
- The following should also be submitted when required and available. This will greatly assist and speed up the application process.
- A copy of your license where one is required.
- A copy of your bond or insurance policy where required.
- A copy of your business letterhead and a business card.
Applications will be acted upon by the Executive Committee after receiving and reviewing the membership committee's investigation and recommendations.
NOTE: Please ensure that all questions are answered in full and that the Waiver and Certification are both signed & dated by the Applicant.

- Application Fee $50.00 (Non refundable)
- First Year's Dues in Advance $195.00
TOTAL: $245.00
